I need the missing items.

I got this quickly, but had to wait to get someone to install it, so the box was not opened until recently. The box was missing several parts: installation instructions, screws to hold the fan blades in place, the 5" down shaft (which I need) and the clips to fasten the crystals to the assembly. Since it took so long to open the box, I need these items in order to have it installed.
