This is a trundle bed. It is meant to go under another twin-size bed and is not meant to be a standalone unit. The mattress I would recommend would depend on whether you are using it as a trundle bed or not. If you are, the mattress can only be a few inches deep and would not be the best for a bad back. If you intend to get a regular twin bed or use this as one, I would suggest item #14024922.
