Does the insert go into the duvet? As in, you do not have to place a comforter into the duvet?

No the duvet is the cover, and you don't have to place a comforter into the duvet.

I have several questions:
What is the thread count of the fabric for this set?
Does the set include a duvet cover and an insert (comforter/duvet)?
Are all of the components cotton fabric, as noted in the description?

The duvet does have an insert of 100% Siliconized polyester fiber fillers, the rest of the material is 100% cotton. Unfortunately, I could not find the thread count.
